Feedback and complaints
You can tell us what is working and what is not. You do not have to give your name, and making a complaint will not, by itself, reduce or stop your supports.
You can contact us any way that works
- Call 0412 004 579 (Monday to Friday, 9:00am–6:00pm).
- Email admin@quantumcare.com.au. Put “Complaint” in the subject if you want.
- Hand the form to a worker, or ask a family member, nominee, or advocate to do it with you.
You do not have to fill in a form. A phone call or an email is enough. If you do not leave a way to contact you, we will still look at what you told us, but we may not be able to tell you the outcome.

This is the Quantum Care feedback and complaints form (QC-F-05). It already has our logo. On a phone, open it and print it, or write on a paper copy and email a photo to admin@quantumcare.com.au. The “Office use only” section is for us. Leave it blank.

On this website the legal name is Quantum Care Solutions Pty Ltd.
What happens next
If you leave a way to contact you, we aim to acknowledge the complaint within 2 business days and explain what happens next. We record it, respond, and review whether something needs to change. We will not treat you differently because you complained.

If something unsafe happens
If someone is in danger, call 000 first. Then call us on 0412 004 579 or email admin@quantumcare.com.au and say it is urgent. We will make sure the person is safe, record what happened, and notify the NDIS Commission when the law requires it.
The staff incident report is not published here. It is an internal record. You do not have to decide whether something is a “reportable incident”. Tell us what happened.

You do not have to complain to Quantum Care first. The NDIS Quality and Safeguards Commission takes complaints about registered and unregistered providers.

An advocate can help you. Using an advocate will not be held against you. Complaints about the NDIA, your plan, or access to the NDIS are a different body: the NDIA on 1800 800 110.
