Quantum Care collects name, contact details and the information you include in an enquiry so we can reply and, if you become a participant, deliver supports. We do not sell personal information.
Enquiry forms are processed by Web3Forms and delivered to admin@quantumcare.com.au. Do not send health records, shift notes, or NDIS plan documents through the public enquiry form.
If the usual rostering system is unavailable, staff may use an unlisted page on this website to send a shift note. That page asks for a staff code. The note is emailed to admin@quantumcare.com.au and is not stored on the website. Use it only for that outage, and do not include an NDIS number.
